If you plan to climb our 30-foot climbing wall while at the center, please bring a completed release form with you. (You may also get one at the center.) We must have a form on file for each person who climbs.

Climbing Wall Release Form

If you or your child is attending a party at the Mason Community Center, please have him or her bring the appropriate release form to the party.
